Telehealth or Telephone Consult

Telehealth consults are easy with a computer, tablet or mobile phone. I send you a link in an email and you click on it at our appointment time. It’s just like we are in the same room. After our consult I email you with resources and a summary of strategies.
Alternately, we can do a telephone consult. The process is the same, it’s just conducted over the telephone.
Cost is $140 for an initial consult, $80 for a review consult.

Home Visit

If you live in the Inner Northern Brisbane suburbs I can visit you at home. The process of the consultation is the same as a telehealth one, it’s just done in person. Your partner or carer can be present if you wish. At the end of the consult I’ll jot down a summary of strategies and email you resources.
Cost is $170 for an initial consult, $110 for a review consult.

A diabetes education consult can help you feel more confident managing diabetes day to day.

In your consult, I’ll help you understand what those numbers mean and what you can actually do with that information.

We can look at patterns in your blood glucose levels, talk through your current routine, discuss your food and activity, and work out practical next steps that feel achievable for you.

No judgement. No lectures. No assuming you should already know everything.

Just clear diabetes education, realistic strategies and support that fits your everyday life.

Support for type 1 diabetes, type 2 diabetes and gestational diabetes

Diabetes management is not one-size-fits-all.

Your support will depend on your type of diabetes, medications, health history, lifestyle, goals and what you are finding difficult right now.

Type 2 diabetes and pre-diabetes support

If you have type 2 diabetes or pre-diabetes, I can help you understand blood glucose levels, food choices, activity, medications, health checks and realistic lifestyle changes.

I can even look at helping you to achieve remission of your diabetes.

We can focus on what matters most, without making you feel like you have to overhaul your entire life by Monday.

Type 1 diabetes support

If you have type 1 diabetes, I can help with blood glucose patterns, hypos, food choices, exercise, ketones, sick day planning and everyday diabetes problem-solving.

Diabetes can be relentless, so support needs to be practical, compassionate and realistic.

What’s included in a diabetes education consult?

Your diabetes education consult may include:
– a review of your diabetes history, current routine and main concerns
– support to understand blood glucose levels and HbA1c
– considering technology available for managing your diabetes
– practical strategies for food, activity and lifestyle
– help with hypos, ketones, sick days and safety planning
– discussion of diabetes medications and how they fit into your routine
– realistic goals and next steps
– resources, recommendations or strategies emailed after your appointment
– booking follow-up appointments to review progress and adjust your plan.

Initial appointments are usually 1 hour, and review appointments are usually 30 minutes.

Appointment options and fees

Diabetes education consults are available by telehealth or phone Australia-wide.
Initial appointment: $140
Review appointment: $80, available after an initial consultation.

Home visits may be available in selected Brisbane suburbs. A home visit costs:
Initial home visit appointment: $170
Review home visit appointment: $110, available after an initial consultation.

Medicare, DVA, NDIS and private health rebates

As an Credentialled Diabetes Educator, I am registered with Medicare, the Department of Veterans’ Affairs, and major private health funds.

Private health rebates vary depending on your level of cover.

To claim a Medicare rebate, you will need to see your GP for a relevant referral, such as a Team Care Arrangement or Chronic Disease Management Plan. Medicare rebates can usually be processed electronically, but appointments are not generally bulk billed.

DVA Health Cards, Gold and White, are accepted with a GP referral.

NDIS may be invoiced through your service provider.
